Through a strategic partnership with the American College of Cardiology (ACC) to deliver personalized medicine, Kansas City-based Health Outcomes Sciences recently announced the availability of the first integrated 30-day PCI readmission model.
 
"Healthcare systems are searching for ways to identify high-risk PCI patients earlier in the care process in the wake of changes in reimbursement for 30-day readmissions," says Harlan Krumholz, MD, SM, who developed the model at Yale University's New Haven Center for Outcomes Research & Evaluation.   

"This model, delivered through HOS’ patented ePRISM software, extends the ability of providers to individualize care from better management of bleeding risks and use of drug eluting stents to now support discharge planning” says John Spertus MD MPH, one of the developers of the ePRISM software.
 
The ePRISM predictive modeling tool takes pertinent patient demographic and clinical information, and models, in real time at the point of care, the patient's risk of readmission. Work continues on integrating models for CHF, AMI, and pneumonia into ePRISM, with delivery expected in the first quarter of 2011.
